Social geriatrics in Laval, a free service.

History

Founded by geriatrician Dr. Stéphane Lemire, the social geriatrics approach in the community aims to preserve the health and autonomy of older adults by addressing a range of health determinants directly in their living environment, with a focus on aging at home. Its objective is to intervene preventively or as soon as early signs of health decline are detected among elders, to prevent accelerated aging that could lead to more intensive care. The Cooperative, a bearer of social innovation

The social geriatrics project, launched in February 2020 in collaboration with the AGES Foundation and the CISSS of Laval, aims to support elders directly in their living environment through a proactive and community-based approach. It mobilizes stakeholders from the health network, social services, and the community to quickly identify needs and prevent the loss of autonomy.

Since the start of the project:

  • Over 1,700 alerts have been received,
  • More than 1,800 sentinel volunteers have been trained,
  • And over 6,400 support actions have been carried out by the social geriatrics team in Laval.

Together, we contribute to developing a caring community around elders.

The counselors play a central role in social geriatrics. They support older adults by identifying their needs, implementing concrete actions at home, and guiding them toward the most suitable resources. Through personalized follow‑up and close collaboration with a wide network of partners and sentinels, they help elders remain at home, safely and with dignity.

The risk of falling is unfortunately a reality faced by many elders. You’ll discover practical ways to prevent them. We will equip you with the knowledge you need to better understand how to stay balanced in your daily life.

LUCI Program for Healthy Lifestyle Habits

The mission of LuciLab is to promote cognitive and physical health by sustainably improving lifestyle habits.

Accessible through a user‑friendly digital platform, the LUCI program is grounded in prevention science. Its human‑centered and technology‑driven approach uses proven behavior‑change techniques to help participants progress at their own pace toward a healthy and protective lifestyle.

Loan of Mobility‑Adapted Equipment

In October 2025, the official launch of the project to loan mobility‑adapted equipment for elders took place in collaboration with several Laval organizations.

Our social geriatrics advisor, Stéphanie Fournier, leads this wonderful project and presented this new free service available in Laval for elders.

Thanks to our partners, Laval’s social geriatrics team supports older adults in maintaining their autonomy and preventing falls by offering free loans of canes, walkers, rollators, transfer benches, and other mobility equipment.

We are fortunate to rely on the valuable collaboration of the CISSS de Laval, the Laval Municipal Housing Office, the Bourassa Savaria Foundation, and the City of Laval.

The AGES Foundation and the Elders Health Innovation Lab (LISA) enable the development of projects throughout Québec to enhance the well‑being of older adults.
